Covid-19 - Business Notification Form

    1. If you or your organisation wishes to start undertaking any of the activities allowed from Monday 22nd February in Guernsey and Thursday 18th February in Alderney, you need to notify Environmental Health and Public Health by completing this online form. If you have any questions or concerns, please email trade@gov.gg.

    15. The next section of the form will ask you to confirm that you have understood and put in place measures to reduce health risks to the public, your customers, yourself and your staff.
    16. If you are unable to tick all of the boxes you will need to contact trade@gov.gg.
    20. For example, Public Health may need to ask you for details of which staff were working on each site and/or which customers were in each controlled environment by day and time of day along with contact details for contact tracing purposes. With this, comes certain data protection obligations, as set out here.
